ACOG implantation physiology and testing
The American College of Obstetricians and Gynecologists notes that implantation bleeding occurs in roughly one in three pregnancies. It is harmless and requires no medical intervention.
Implantation triggers cellular production of human chorionic gonadotropin (hCG). Serum hCG doubles roughly every 48 hours during early pregnancy.
While sensitive digital home tests detect hCG at concentrations of 10 to 25 mIU/mL, testing before 12 DPO carries a significant false negative rate because implantation may have occurred on day 10 or 11.
Worked example: Implantation and test sensitivity
For ovulation on October 1st, the peak implantation day is October 10th (9 DPO).
An ultra-sensitive 10 mIU/mL test may detect pregnancy by October 13th (12 DPO), while a standard OTC test reaches 99% accuracy on October 15th (14 DPO).

CALCULATOR QUESTIONS
ACOG embryo implantation timeline calculator questions
What causes implantation bleeding?
When the blastocyst burrows into the vascular endometrium (uterine lining), it can rupture minute maternal capillaries, causing slight light pink or brown spotting.
Can stress delay implantation?
Severe acute physiological stress can delay ovulation, but once ovulation occurs, the timing of fertilization and subsequent blastocyst migration is biologically autonomous.
